Specific health checkup (using a checkup ticket)

This basic checkup focuses on identifying metabolic syndrome.
A checkup ticket is required for this checkup. See the procedures to request to have a checkup ticket sent to you.

Eligible persons

Employee family members, Voluntarily and Continuously Insured Persons and their family members, and Special-Case Retired Insured Persons and their family members aged 40 years or older who are members of the Hitachi Health Insurance Society

• *Subsidies are available only to those who are members of the Hitachi Health Insurance Society at the time of the checkup.
• *The age used to determine eligibility is the age as of the end of the fiscal year (March 31).

Number of times the subsidy can be provided

Once per fiscal year

• *The subsidy is available once per fiscal year for one of the following checkups: complete medical checkup; abbreviated complete medical checkup; health checkup for women; specific health checkup (using a checkup ticket).
• *The subsidy for a health checkup is paid only once per fiscal year. This applies even if you left your employer during the fiscal year but remained a member as a Voluntarily and Continuously Insured Person, a Special-Case Retired Insured Person, or a dependent

Free (full cost paid by the Health Insurance Society)

Examining institutions where you can undergo this checkup

Only group contracted examining institutions
Find group contracted examining institutions by any of the following methods:

• Contact your municipality (regarding residents’ health checkups or related matters).
  • *You may be asked to provide the name of the contracting organization. For the Hitachi Health Insurance Society, answer "NFHIS Group A, Group B."
• Contact the Hitachi Health Insurance Society Health Checkup Call Center.

• Search the list of contracted examining institutions.

  • *The list of contracted examining institutions currently available is the FY 2023 edition.The list of contracted examining FY 2024 edition will be available in late July 2024.

Notes

• Be sure to bring the checkup ticket with you on the date of the checkup. You will not be able to undergo the checkup without the ticket.
• If you are getting a specified site examination (optional examination) at the same time, pay the full cost of the specified site examination at the examining institution counter and obtain a receipt. Claim the subsidy from the Hitachi Health Insurance Society for examinations eligible for a subsidy at a later date.

• Detailed examination items of the specific health checkup (i.e., anemia testing, electrocardiogram, funduscopy) must be added by a doctor. They cannot be requested by examinees themselves.
  Examinees must pay the full cost of any additional examinations that they request themselves.
