After you leave your employer

After leaving your employer, you will lose your eligibility for membership in the Health Insurance Society and must join the appropriate medical care insurance program based on your individual circumstances.

Returning health insurance cards

Documents to attach
Eligibility to join Please check the color and symbol of your health insurance card. Click here for an image of the health insurance card.	Insured person (employee) If the symbol starts with "1".
Application Form	Health insurance card Cards for insured person and all dependents (See here if the health insurance card is lost and cannot be returned.)
Deadline	Within five days from the date of loss of eligibility
Submit documents to	Your employer’s health insurance contact (e.g., General Affairs, Labor Management)
Others	If a health insurance card has been used to undergo examination or treatment at a medical care institution or other such facility on or after the date of loss of eligibility (i.e., the day after the date of separation), the insured person will be billed for the portion paid by the Hitachi Health Insurance Society as unqualified examination or treatment costs. Promptly return all health insurance cards after loss of eligibility to prevent unintended use.
